General information clause

IZOSTAL S.A., pursuant to Article 13 of the General Data Protection Regulation of 27 April 2016 (OJ EU L 119 of 04.05.2016) - hereinafter GDPR, informs that:
1) The Controller of your personal data is IZOSTAL S.A., ul. Opolska 29, 47-113 Kolonowskie.
2) The Company has appointed Ms Aleksandra Szyszka-Schuppik as Data Protection Supervisor, who can be contacted at the following e-mail address: rodo[at]izostal.com.pl.
3) Your personal data are or may be processed:
- in order to implement an agreement or to take action before it is implemented or concluded (Article 6(1)(b) GDPR)
- in order to fulfil the Controller's legal obligation (Article 6(1)(c) GDPR)
- in the Controller's legally justified interest (Article 6(1)(f) GDPR) to ensure the necessary settlements in connection with the concluded agreement, the assertion of claims, direct marketing and relationship and building relations with Customers,
- on the basis of voluntary consent (Article 6(1)(a) GDPR)
4) In accordance with the GDPR you have the right to:
- access your data and to receive a copy thereof,
- request correction (rectification) of personal data - when you notice that the data is incorrect or incomplete,
- delete the data if there are no grounds for further processing,
- request to restrict processing of personal data,
- objection in the case of processing of personal data for marketing purposes or due to special circumstances,
- lodge a complaint to the President of the Personal Data Protection Office.
5) You have the right to withdraw your consent at any time with no impact on the lawfulness of the processing, which was made on the basis of consent prior to its withdrawal, and you are entitled to transfer your data.

Clause used in recruitment

I consent to the processing by IZOSTAL S.A., ul. Opolska 29, 47-113 Kolonowskie, of my personal data included in the application documents sent by me in order to participate in future recruitments conducted by IZOSTAL S.A.
I declare to have been informed that:
- I have the right to withdraw my consent at any time by sending an e-mail to info@izostal.com.pl or by sending a letter to the address of the Personal Data Controller above,
- the withdrawal of consent does not affect the lawfulness of the processing of my personal data that was carried out before its withdrawal.

 

Legal notice – contact form

IZOSTAL S.A., pursuant to Article 13 of the General Data Protection Regulation of 27 April 2016 (OJ EU L 119 of 04.05.2016) - hereinafter GDPR, informs that:
1) The controller of your personal data is IZOSTAL S.A., ul. Opolska 29, 47-113 Kolonowskie.
2) The Company has appointed a Data Protection Officer, Ms. Aleksandra Szyszka - Schuppik, who may be reached at the following e-mail address: rodo[at]izostal.com.pl.
3) Your personal data will be processed for the purpose of establishing contact (Article 6(1)(f) GDPR - legitimate interest of the Controller).
    4) The recipients of your personal data will be:
    a) entities entitled to obtain personal data under the law,
    b) entities to which the Company has entrusted the processing of personal data,
    c) corporate affiliates,
    5) Your personal data will be kept for a maximum of 2 years. The period of processing may be extended where processing is necessary to establish, pursue or defend against possible claims.
    6) You have the right to demand from the Controller access to your personal data, the right to rectify, delete or restrict the processing. You have the right to revoke your consent at any time without affecting the lawfulness of the processing carried out on the basis of your consent prior to revocation and you have the right to data transfer.
    7) You have the right to lodge a complaint with the Office for Personal Data Protection ul. Stawki 2, 00-193 Warszawa.
    8) The provision of personal data is necessary to establish contact.
    

 

Legal notice for civil law contracts and registered businesses

IZOSTAL S.A., pursuant to Article 13 of the General Data Protection Regulation of 27 April 2016 (OJ EU L 119 of 04.05.2016) - hereinafter GDPR, informs that:
1) The controller of your personal data is IZOSTAL S.A., ul. Opolska 29, 47- 113 Kolonowskie.
2) The Company has appointed a Data Protection Officer, Ms. Aleksandra Szyszka - Schuppik, who may be reached at the following e-mail address: rodo[at]izostal.com.pl.
3) The personal data transferred will be processed for the purpose of:
- conclusion and implementation of an agreement - pursuant to Article 6(1)(b) GDPR,
- financial accounts, including tax settlements, under Article 6(1)(c) GDPR,
- necessary to establish, assert or defend claims, including damages, related to the above mentioned agreement, as well as to answer requests, questions or complaints and to respond to pending proceedings (legitimate interest of the Controller), pursuant to the provision of Article 6(1)(f) GDPR.
4) The recipients of your personal data will be:
- entities entitled to obtain personal data under the law,
- entities to which the Company has entrusted the processing of personal data,
- corporate affiliates,
- entities to which the Company provides services,
- banks - in terms of payment processing,
- postal operators and companies providing courier services,
- occupational medicine service.
5) Your personal data will be stored for 50 years with respect to commission agreements with paid pension contributions (Act of 17.12.1998 on pensions and retirements) or 10 years for persons who have established cooperation after 31.12.2018. - Act of 10 January 2018 on amending certain acts in connection with the shortening of the period for retaining employee files and their digitization.
6) You have the right to access the content of your data and the right to rectify, erase and restrict the processing of your data, as well as the right to transfer data, the right to object, the right to withdraw consent at any time without affecting the lawfulness of processing carried out on the basis of consent prior to its withdrawal (if the processing is based on a consent).
7) You have the right to lodge a complaint with the Office for Personal Data Protection, ul. Stawki 2, 00-193 Warszawa.
8) The provision of personal data is obligatory under the provisions of law, and in the remaining scope it is voluntary.

 

Information clause for e-mail correspondence

IZOSTAL S.A., pursuant to Article 13 of the General Data Protection Regulation of 27 April 2016 (OJ EU L 119 of 04.05.2016) - hereinafter GDPR, informs that:
1) The Controller of your personal data is IZOSTAL S.A., ul. Opolska 29, 47-113 Kolonowskie.
2) The Company has appointed Ms Aleksandra Szyszka-Schuppik as Data Protection Officer, who can be contacted at the following e-mail address: rodo[at]izostal.com.pl.
3) Your personal data will be processed for the purpose of:
- enabling e-mail contact with the Controller and communication with the addressees,
- documenting arrangements made with customers, contractors and other persons,
- receiving letters, notifications and requests in electronic form, e.g. complaints, claims, other requests,
- protection against claims and enforcement of possible claims.
4)  Your personal data will be processed pursuant to:
- the legitimate interest of the Controller (Article 6(1)(f) of the GDPR).
- the need to implement the contract concluded between the parties (Article 6(1)(b) of the GDPR),
- consent given through informed action (Art. 6(1)(a) of the GDPR).
5) The recipients of your personal data will be:
- entities entitled to obtain personal data under the law,
- entities to which the Company has entrusted the processing of personal data,
- corporate affiliates.
Your personal data will be retained for a period necessary to assert claims by the Controller in connection with the conducted activity or to defend against claims made against the Controller, on the basis of generally applicable laws, taking into account the periods of limitation of claims specified in generally applicable laws.
6) You have the right to access the content of your data and the right to rectification, erasure, restriction of processing, right to data portability, right to object, right to withdraw consent at any time without affecting the lawfulness of the processing carried out on the basis of consent prior to its withdrawal, and the right to data portability.
7) You have the right to lodge a complaint to the Personal Data Protection Office, ul. Stawki 2, Warsaw.
8) Provision of the personal data is voluntary, but it is necessary for obtaining a reply to the sent correspondence.

 

Legal notice for persons specified in contract – party representatives

IZOSTAL S.A., pursuant to Article 13 of the General Data Protection Regulation of 27 April 2016 (OJ EU L 119 of 04.05.2016) - hereinafter GDPR, informs that:
1) The controller of your personal data is IZOSTAL S.A., ul. Opolska 29, 47- 113 Kolonowskie.
2) The Company has appointed a Data Protection Officer, Ms. Aleksandra Szyszka - Schuppik, who may be reached at the following e-mail address: rodo[at]izostal.com.pl.
3) Your data will be processed with respect to: name, surname, job title, company telephone number and e-mail address.
4) Your personal data will be processed for the purpose of:
- preparation, conclusion and implementation of an agreement - pursuant to Article 6(1)(b) GDPR
- establishing and pursuing claims or defence against claims of the other Party in relation to an agreement - pursuant to Article 6(1)(f) GPDR,
- archiving to the extent necessary to fulfil legal obligations, in particular those arising from tax regulations, accounting regulations - for the period resulting from these regulations - pursuant to Article 6(1)(f) GDPR,
- in the Controller's legally justified interest (Article 6(1)(f) GDPR) to ensure the necessary settlements in connection with the concluded agreement, the assertion of claims, direct marketing and relationship and image building,
- in the case of voluntary and optional consents, the data will also be processed for marketing purposes consisting in providing you with information about the products sold by the Controller, promotions, price lists and other information, as well as promotional events, trainings and other activities of the Controller, also on the basis of separate consents by means of messages sent to your e-mail address or telephone number and during telephone conversations with you - until the relevant consent is revoked (Article 6(1)(a) GDPR).
5) To the extent that data are processed on the basis of separate consent, you have the right to withdraw your consent at any time. However, this will not affect the lawfulness of the data processing that took place before its withdrawal.
6) The recipients of your personal data will be:
- entities entitled to obtain personal data under the law,
- other entities which, on the basis of relevant agreements signed with IZOSTAL S.A., process personal data, e.g. providing IT, legal, tax, accounting services, auditors, facility security.
7) Your data will be retained for a period:
- necessary for the conclusion and effectiveness of the agreement,
- necessary to assert claims by the Controller in connection with the conducted activity or to defend against claims made against the Controller, on the basis of generally applicable laws, taking into account the periods of limitation of claims specified in generally applicable laws.
8) According to GDPR, you have the right to:
- access your data and receive their copy,
- request the rectification (correction) of personal data - in case the data is incorrect or incomplete,
- delete the data if there are no grounds for further processing,
- demand a restriction on the processing of personal data,
- transfer your data,
- object to the processing of personal data for marketing purposes or because of a particular situation,
- lodge a complaint with the Office for Personal Data Protection ul. Stawki 2, 00-193 Warszawa.
9) Your data comes from the Employer on behalf of whom you are acting in connection with the performance of the agreement.
It is also possible to include the following provision in the agreement:
"A Party shall inform a member of its Personnel of the release of his or her official data to the other Party to the extent necessary to exempt the receiving Party from the information obligation under Article 14 GDPR.”
